About the Role
This is a rare opportunity to be the first clinical hire at a growing care navigation program. You won't just build the program — you'll live it. In this dual role, you'll carry an active caseload of patients while working alongside leadership to design the infrastructure, workflows, and standards that will define how we deliver care at scale. As the program grows, so will you — with the potential to grow into a leadership role overseeing a multidisciplinary team of advocates, CHWs, and clinical staff.
