We are searching for a Pharmacist to join the team! The Pharmacist for Pharmacy Outreach&nbsp;will be responsible for enhancing patient care through telephonic outreach assessments and interventions. This role will focus on activities such as polypharmacy, medication adherence, drug-drug interactions, high-risk medication prescribing, pharmacotherapy optimization, and disease state management. \n Essential Duties &amp; Responsibilities:<ul><li>Identify and address patient-specific barriers to medication adherence and disease state management.</li><li>Prevent, detect, and resolve medication-related problems and make appropriate evidence-based, patient-centered medication and therapy recommendations</li><li>Collaborate with providers, clinical and administrative staff to optimize the Medication Management workflow and patient experience</li><li>Adjust and recommend alternative pharmacotherapy based on formulary changes.</li><li>Assist providers, care managers, or other members of the practice team with management of high-risk, high-cost patients to ensure optimal patient care.</li><li>Other duties as assigned&nbsp;</li></ul> Required Skills and Abilities: <ul><li>Strong communication skills with patients, physicians, and other healthcare professionals</li><li>In depth knowledge of pharmacotherapy and pharmacy practice</li><li>PGY1 pharmacy residency</li></ul> Required Education &amp; Experience:<ul><li>Graduate of an accredited School of Pharmacy</li><li>Doctor of Pharmacy (PharmD) </li><li>1+ year of experience in a clinical pharmacy setting</li><li>Current and valid multi-state licensure including at least 2 of the following states: AR, DE, OH, KY, MS, PA, WA, WV </li><li>Willingness to gain up to 3 additional licenses within 90 days of hire</li></ul> Preferred:<ul><li>Licensure in 4+ of the above states; BCACP</li></ul> \nWho We Are:Aledade, a public benefit corporation, exists to empower the most transformational part of our health care landscape - independent primary care.
