Summary
The Project Coordinator (PC), Practice Transformation, provides administrative and coordination support to our Practice Transformation teams. The PC serves on a team dedicated to providing technical assistance to our external healthcare organization (HCO) partners and reports to the Senior Director of Practice Transformation or other supervisors, as assigned. The PC is responsible for overall coordination and organization for the team, and for individual HCO engagements within their team. This includes meeting scheduling and HCO-training logistics, minute taking for critical HCO-facing meetings, logging of data into Salesforce, and managing the distribution of materials to our HCO partners
Essential Duties and Responsibilities
Serve as the project coordinator of practice transformation activities and key integrator of information and process intended to drive the delivery of high quality support to healthcare partners.&nbsp;
Key responsibilities include:&nbsp;
<ul>
<li>Convert received content on HCO activities into standardized status reports&nbsp;</li>
<li>Enter HCO implementation notes, communications, and resources into various data and technical platforms (e.g. Salesforce and Asana)&nbsp;</li>
<li>Drive scheduling/coordinating larger meetings for HCO-facing work</li>
<li>Coordinating various steps and logistics to support HCO partners through the Upstream Training process&nbsp;</li>
<li>Purchase and manage the distribution of materials to HCOs, including, Patient Education Materials, appreciation gifts, and supplies, as needed&nbsp;</li>
<li>Enter HCO related expenses and grants into budgets&nbsp;</li>
<li>Support data-driven quality improvement through developing expertise in available data platforms, including Azara, IMAT and Salesforce, and retrieving and validating reports for the PT team, leadership, and HCO partners&nbsp;</li>
<li>Develop knowledge and gain HCO facing experiences in supporting key meetings such as the program kick-off meeting.&nbsp;</li>
<li>Support PTL in planning for and managing HCO-site visit logistics</li>
<li>Additional duties as assigned</li>

Required Education, Experience, Knowledge, Skills and Ability

<li>Excellent verbal and written communication skills with ability to work independently and collaborate with timely coordination with other team members</li>
<li>Demonstrates effective multitasking, and organizational skills with keen attention to detail to complete competing priorities in timely manner</li>

<li>In our hybrid work environments, there is a basic expectation that our Upstreamers will ensure that their work from home setups will have reliable access to phone and Internet to ensure connectivity to their teams.</li>
<li>At Upstream we gather for moments that matter for training, teaming and connection. Our teams come together for occasional in person meetings and organizational retreats. As part of our hybrid work practices, this travel expectation will be applicable for all Upstreamers, even those based remotely.</li>
<li>Engage in EDI learning &amp; development, community engagement, and culture building activities up to 5% FTE (i.e., 2 work hours per 40 hour work week). Examples include attending trainings, conferences and summits, volunteering time, participating in local community events, mentoring and learning, participating in Communities of Belonging or Culture Council, developing language acquisition, learning universal design principles, and using professional development benefits for EDI topics, among other activities.</li>

Travel Requirements
This role will require up to 35%&nbsp; business travel. All Upstreamers can expect a minimum of 8 days of business travel per year to attend 1-2 annual organizational retreats and two annual team/department meetings.
